> An online COVID-19 Screening form is required to be filled by each patient prior to every appointment.
> We make sure to sanitize all areas of the clinic after each patient visit.
> All patients and staff must wear a face mask at all times.
> A limited number of patients are allowed in the clinic at one time.
> No walk-in appointments are available at this time.
